- Form design and artworkYou bring the questions; we combine professional graphic design with specialist form layout, tailored to your branding, respondents and scanning requirements. Supporting artwork can be included as required.
- Printing and personalisationPrinted in Australia to the scanning specifications, in full colour with dropout-ink response areas, personalised with IDs, barcodes, QR codes and name-and-address merge. Guaranteed to scan.
- Distribution and returnsIndividual mail-outs and bulk delivery through Australia Post and couriers, with completed forms returned directly to CDS or received in bulk from your organisation and participating sites. Return reporting and reminders can be included as agreed.
- Scanning and data captureClosed responses read by optical mark recognition; critical fields, blanks and double marks checked by an operator; open-ended answers keyed. Manual entry is an alternative for smaller runs.
- Data delivery and return reportingExcel or CSV with agreed column names, linked PDFs for scanned forms and fieldwork return counts. Images for manually keyed projects are optional at extra cost.
